BOSSnet e-Forms Summary

For Core Medical Solutions, BOSSnet e-Forms represent an exciting pathway for future product development towards to a usable Electronic Medical Record.
For existing BOSSnet DMR users who are comfortable with the retreival of scanned documentation within BOSSnet DMR, e-Forms present the next progressive step by way of a powerful yet intuitive transcription tool which maintains user familiarity and therefore confidence, comfort and competence in using BOSSnet as a functional EMR.

Within a BOSSnet DMR site, BOSSnet e-Forms empower those user groups ready to progress, to do so without having to wait for the entire organisation to become ready.
In fact, the Psychiatric Services Department at Ballarat Health Services converted each of their MR forms to e-Forms within just 6 weeks of initial BOSSnet deployment, whilst todate many other departments across BHS maintain their paper forms and post-discharge scanning processes.

BOSSnet e-Forms employ the same cognitive processes undertaken by clinical users when recording clinical information by pen and paper, making training and change management much less daunting.

e-Forms become immediately viewable post-submission by the author, or can be saved in draft form ready to be completed and published later.

Unlike paper MR forms, e-Forms permit automated audit and searchability across all levels of user and administrator.

Some pieces of clinical data are inefficiently entered multiple times on paper MR forms. BOSSnet e-Forms enable configuration of auto-populated data fields which appear on multiple forms containing the same data.

BOSSnet e-Forms are classified as:

  1. Simple: designed, approved and managed by the client.
  2. Advanced: mini web applications designed and managed by Core Medical Solutions.
  3. eDoc: a page displayed within BOSSnet DMR summarising data recorded in another BOSSnet application.

SIMPLE e-FORMS

In their most basic guise, Simple e-Forms can be a scanned image of a MR form.
System Administrators are able to use Microsoft Word, Open Office or Adobe LiveCycle to edit the scanned image to include user-populatable data fields (which are given names corresponding to specific BOSSnet database fields).
Some of these data fields correspond to the sections of the MR form which a user would usually write information in, and can comprise of any combination of free-text, check box and pull-down menu.
Other data fields correspond to objects already located in the BOSSnet database, and can be configured to auto-populate as text or barcodes (eg patient demographics).
The template is then added to the e-Form registry in BOSSnet Admin, and made available to the relevant user groups.

 

A degree higher in complexity is the custom-built Simple e-Form.
This is built from scratch by client System Administrators (again using MS Word, Open Office or Adobe LiveCycle), and is a fantastic way to enhance the physical appearance and online functionality of your existing forms catalouge, all in-house.
Once again, input data fields can be any combination of free-text, check box and/or pull down options (see Epworth Cardiac Device Implant Information example).
Simple e-Forms can be deployed in a fashion which fills an intermediary uptake gap within a user group transitioning towards electronic transcription. The BOSSnet Progress Note e-Form is able to be populated by the clinical user within BOSSnet, or is able to be printed en-masse by department managers ready for clinicians to write on.
The significant benefit of writing on a pre-printed BOSSnet Progress Note, is that scanning becomes largely automated as every single element of required indexing metadata is guaranteed to be already present and accurate on the form's 2D barcode - Barwon Health and Ballarat Health Services have been able to save as much as 3 hours each day by scanning pre-printed Outpatient Progress Notes in single batches rather than conventional Progress Notes (see Ballarat Health Services Allied Health Progress Note).

 

The BOSSnet e-Form Drawing Tool provides clinicians with the ability to communicate their thoughts pictorially using functionality similar to MS Paint.

System Administrators can upload any number of medical specific drawing templates (lung fields, kidneys, cross sections etc) to make the clinical illustration process simpler and more accurate.

ADVANCED e-FORMS

Advanced e-Forms are so termed, as they are offer inbuilt intelligence and complexity not achievable by a Simple BOSSnet e-Form, or paper form for that matter.

Advanced e-Forms are able to include mandatory fields, decision support, real-time analysis of input, multiple form sections and the ability to revisit the form over and over again.

Core Medical Solutions have a growing library of Advanced e-Form templates including:

  • Discharge Summaries
  • Operation/Procedural Notes
  • Internal Referrals

e-DOCUMENTS

An e-Doc allows clinical users to view a page in the DMR which summarises data recorded elsewhere in BOSSnet, meaning the user does not have to leave the DMR to view information such as:

  • Paediatric Growth Chart
  • Handover Note Summary page
  • Observation Chart
